Thanks for your answer, you are right they are label but when I change the template to Beez3 - it show the registration form or the guest form (images attached)

I have checked your site and figured out that it works on Bootstrap 2, when you switch to T3 blank template, it works properly as the markup is from Bootstrap 2. To make it work in Bootstrap 3 (that means it will work withy T3 bs3 blank template), please chang the markup from:

<div class="btn-group btn-group-vertical" data-toggle="buttons-radio">
